Course Fee Application Dynamic Form Instructions

This guide provides step-by-step instructions on how to fill out the Course Fee Application form. It includes details on selecting the appropriate term, course, fee type, and amount, as well as how to sign and submit the form. Following this guide will ensure a smooth and accurate submission of the application.

 

1. Open the Course Fee Application Dynamic Form

 

 

2. Select the appropriate term in the "Term Code" selection list. 

 

 

3. Type in the course CRN

 

 

4. Open the “Type” list
 

 

5. Select the appropriate fee type from the type list

 

 

6. Open the “Detail Code” list

 

 

7. Select the appropriate fee from the list

 

 

8. Open the "Fee Type" list

 

 

9. Select the appropriate "Fee Type" from the fee type list 

 

 

10. Enter the fee amount in the “Amount” field

 

 

11. In the "Justification of Change" field, enter your justification

 

 

12. Select “Click to sign” to enter your signature

 

 

13. Enter your first and last name in the appropriate fields

 

 

14. Select “Sign Electronically” to complete your submission

 

 

15. Select "Submit Form"

 

 

16. Your form will then be routed to the appropriate reviewer/approvers in your department along with the Provost and Student Business Services
