This guide provides step-by-step instructions on how to fill out the Course Fee Application form. It includes details on selecting the appropriate term, course, fee type, and amount, as well as how to sign and submit the form. Following this guide will ensure a smooth and accurate submission of the application.
1. Open the Course Fee Application Dynamic Form

2. Select the appropriate term in the "Term Code" drop down selection box.

3. Type in the course CRN

4. Click on the "Type" drop down box

5. Select the appropriate fee type from the drop down selections

6. Click on the "Detail Code" drop down box

7. Select the appropriate fee from the drop down selections

8. Click on the "Fee Type" drop down box

9. Select the appropriate Fee Type from drop down selections

10. Enter the fee amount in the "Amount" box

11. Type "Enter Justification Here"

12. Click "Click to sign" to submit your signature

13. Type in your first and last name in the appropriate boxes

14. Click "Sign Electronically" to complete your submission

15. Click "Submit Form"

16. Your form will then be routed to the appropriate reviewer/approvers in your department along with the Provost and Student Business Services
